I wish to inform you that I will be closing the office located at 5088 NW 74th Avenue, Miami, FL, effective January 31, 2025. I will be available to provide behavioral health care until January 15, 2025.

 

It is very important that you make arrangements as soon as possible to select a new behavioral health provider to make sure you receive uninterrupted mental health care. Please contact your medical plan to obtain a list of physicians in your area that can continue with treatment and overall care.

 

To receive a copy of your medical records, a HIPAA release of information form must be duly executed. The medical records will be available fastest in a digital format posted onto the patient portal prior to January 15th, 2025. 

 

Starting January 31, 2025, the records custodian will be Clary Document Management, Inc. and copies of the record may be obtained by contacting them directly at 763.548.1330. You may also fax Clary at 763.548.1325 or by email at info@clarydm.com.
